We are looking for a Social Worker to support young people and adults as they move from children’s services into adult care. About you

• Hold a social work degree.
• Right to work in the UK and hold clean UK driving license.
• Registered with SWE
• Post qualified experience within children and family services.

Key Responsibilities

• Conduct comprehensive assessments and create tailored transition plans.
• Provide guidance and advocacy to young people and their families throughout the process.
• Collaborate with multidisciplinary teams, including education, health, and social care professionals.
• Ensure compliance with safeguarding policies and care legislation.
• Maintain accurate records and provide detailed reports to inform decision‑making.
• Promote independence, inclusion, and person‑centred care approaches.
